This is an education verification form for the School of International Liberal Studies （SILS） and 

the Graduate School of International Culture and Communication Studies （GSICCS）, Waseda University.

【Required Documents / 必要書類】

A) A copy of the applicant’s valid ID / 対象者の有効な身分証明書（ex: Passport/パスポート）

B) An authorization form signed by the applicant / 対象者の署名入り承諾書

C) Certificate with the issuance number / 発行番号入り証明書　

（ex: Certificate of Graduation・Completion / 卒業証明書・修了証明書）

* A diploma is not verifiable because it doesn’t have an issuance number on it.  

学位記は発行番号が付記されておりませんので、受け付けることができません。

Before applying, please check if the applicant has graduated/is expected to graduate from

the School of International Liberal Studies or the Graduate School of International Culture and Communication Studies, Waseda University.
*Otherwise, we cannot conduct verification.

申請される前に、対象者が国際教養学部・国際コミュニケーション研究科を卒業（見込）・修了（見込）であることをご確認ください。

※それ以外の場合、学歴照会はできません。
